2010 Forms


BSA Medical Form

This form should be used by current members of Nentico Lodge in order to comply with national guidelines. This medical form should accompany you at every Nentico Lodge event. This form is good for one year. You are required to complete parts A and C.

2010 Dues Renewal and Membership Transfer Form

This form should be used by current members of Nentico Lodge who wish to renew their dues. This form may also be used by members in the Order of the Arrow who wish to transfer membership from another lodge into Nentico.

2010 Ordeal Candidate Letter

This is the letter sent to all newly elected Ordeal candidates explaining the Ordeal process, times to report to camp, and fees for the weekend. Dates for Ordeals may be found on the Ordeal/Brotherhood Registration Form. Each candidate should receive this letter at the conclusion of the call-out ceremony.

2010 Ordeal/Brotherhood Registration Form

This form should be used by members of the BSA who were elected by their unit and need to register to attend an Ordeal during the 2010 calendar year. This form may also be used by members of the Order of the Arrow who would like to complete their induction process by taking their Brotherhood at an Ordeal.

2010 Sabbath-Observant Ordeal Registration Form

This form should be used by current members of the BSA who have been elected to the Order of the Arrow and wish to attend an Ordeal, but for religious or personal reasons cannot attend a weekend Ordeal experience. This Ordeal takes place from Sunday evening until Tuesday morning.

2010 Lodge Event Registration Form

This form should be used by members of Nentico Lodge who wish to register for the various events hosted by Nentico Lodge throughout the 2010 year. This form does not include all events as some may require a separate registration form found on this page.

2010 Section NE-6A Conclave Registration Form

This form should be used by current members of Nentico Lodge who wish to register for the 2010 NE-6A Conclave from May 21-23 at Camp Sinoquipe in Fort Littleton, Pennsylvania. The deadline for registering is May 1, 2010.

2010 Nentico Lodge Camping Award Form

This form should be used by the Troops and Teams in Baltimore Area Council who have completed the requirements for the Camping Award. The Camping Award should be completed and mailed to Nentico Lodge no later than December 8, 2010. The Camping Award is distributed to the receiving units at the Annual Lodge Banquet.

2010 Chapter Achievement Program Form

This form should be used by the chapters of Nentico Lodge to report the years accomplishments. It should be completed by the Chapter Chief and Chapter Adviser and turned into the Lodge Chief or Lodge Adviser by the LLDC on December 4, 2010.

Vigil Honor Nomination Form

This form should be used by a member of Nentico Lodge who wishes to nominate a deserving arrowman for the highest award that a lodge may bestow upon its members. The nomination form needs to be submitted by Friday evening of the Spring Fellowship.

Founder's Award Nomination Form

This form should be used by current members of Nentico Lodge who wish to nominate a deserving arrowman for the Founder's Award. The Founder's Award was created to recognize those who give outstanding service to the Lodge. The Founder's Award Nominating Form needs to be submitted by Friday evening of the Fall Fellowship.

Nentico Lodge Name Tag Ordering Form

This form should be used by the Chapters of Nentico Lodge who wish to order name tags for the Chapter Officers.
