2008 Lodge Dues/Membership Transfer Form
This form should be used by members of Nentico Lodge 12 who have not received a dues renewal letter to pay their annual dues, or by a member of another lodge to transfer their membership to Nentico Lodge 12.

2008 Ordeal/Brotherhood Weekend Registration Form (Form-fill) (564 KB)
This form should be used by a candidate elected by their unit to register for one of the many Ordeal weekends held throughout the year. It can also be used by existing members to register to either serve on Ordeal staff or to take their Brotherhood.

2008 Ordeal Candidate Letter
This form is the letter sent to all newly elected Ordeal candidates explaning the Ordeal process, times to report to camp, and fees for the weekend. Dates for Ordeals may be found on the Ordela/Brotherhood Weekend Registration Form.

These letters introduce eligible Brotherhood candidates to the dates, fees and locations for oppurtunities to take their Brotherhood conversions.

2007 Nentico Camping Award Application (Form-fill) (304 KB)
This form should be used by a unit to apply for the Nentico Camping Award which is presented each year at the annual Lodge Banquet.

2007 Chapter Achievement Program (Form-fill) (2.1 MB)
This form is for use by chapter chiefs and their advisors to report the years accomplishments and is due at the Lodge Leadership Development Conference.
